Membership Terms and Conditions
- Membership Overview
The At Home Dental Care Membership provides four visits per year, with dental care delivered either at the patient’s home or in a care home, depending on the membership you are joining. The pricing and services are as follows:
- Care Home Residents: £23.75 per month
- Private Home Patients: £47.50 per month
Each visit includes an exam/review and scale and polish if clinically necessary. Any treatment identified during the initial exam will be eligible for a 10% discount and will be fixed. Going forward, any additional identified treatment needs will be eligible for up to a 50% discount.
- Included Services
The membership includes four visits per year, with each visit covering the following services:
- Exam/review (x-rays included in first visit)
- Scale and polish (if clinically necessary).
Additionally, the membership provides the following discounts on treatment:
- 10% off any treatment identified during the initial exam.
- 50% off any further treatment identified during subsequent visits, including fillings and extractions.
- 25% off dentures or crowns (or any similar treatment, involving laboratory work).
- 25% off root canal treatment.
- Service Fee
There is no service fee for the regular four visits per year (the scheduled visits every 12 weeks).
For any additional appointments outside of the regular visits (e.g., unscheduled visit, follow-ups), a discounted service fee of £28 will apply (excluding urgent appointments which will remain at full price £56) - Treatment Limitations and Referrals
In the event that any treatment is not possible due to lack of cooperation, out of the scope of practice for the home setting, or deemed too difficult to perform safely, we reserve the right to refer the patient to a third-party dental provider.
- No discounts will apply to treatment provided by the third party.
- If a patient is unable or unwilling to cooperate during a visit, it will be at the discretion of our team whether we attempt to provide treatment on a subsequent visit.
- Commitment and Cancellation
The membership requires a minimum commitment of 6 months from the registration date.
After the initial 6-month period, you may cancel the membership by providing one month’s notice. Cancellations must be submitted in writing. If you choose to terminate the membership within 14 days of the registration date you will be charged a private fee for each completed appointment. If you choose to cancel the membership within 6 months of the registration date you will be charged for the first 6 months in full (less any paid membership fees)
If the patient’s resident status changes (e.g., from private patient to care home, etc.), the patient will be eligible to change membership (move onto the care home membership).
- Appointment Scheduling and Notifications
Upon agreement of proceeding with the membership, you will be scheduled for your first appointment.
- Once you have agreed to the membership, your first appointment will be scheduled. The date of this appointment will depend on capacity.
- If your first appointment is within 14 days and you choose to terminate within this time period you will be invoiced the private fees that are applicable.
- The membership includes visits approximately every 12 weeks, but this may vary due to logistical issues. However, we will attend four times a year.
- If we are unable to visit four times per year due to patient illness, lack of cooperation, or any access-to-care-home issues, there will be no entitlement to a refund.
- Additional Treatment and Referrals
If additional treatment is not possible in the care home, or if a referral to another dental specialist is needed, the cost for this treatment or referral will not be included in the membership.
If treatment cannot be provided at the care home (e.g., due to the nature of the care environment), additional arrangements will be made for follow-up care at a different location or time, at the patient’s expense. - Patient’s Responsibility for Medical Information
It is the responsibility of the patient (or their caregiver) to provide accurate and up-to-date medical history and any information regarding current medical conditions, medications, or allergies that may affect treatment. Failure to provide complete information could result in the postponement or modification of the treatment membership. - Data Protection and Confidentiality
We are committed to protecting your personal and medical information. All personal and health data will be treated with the utmost confidentiality in accordance with applicable data protection laws (such as GDPR). Your information will only be used for the purposes of providing dental care and will not be shared with third parties without your consent, except as required by law. - Feedback and Complaints
We value your feedback. If you have any concerns or complaints about your experience with our service, please contact us at reception@homedental.org so we can address them promptly. We are committed to continuously improving our services and value your input. - Emergency Service Disclaimer
We will do our utmost to accommodate your needs and provide timely care. However, please note that we are not an emergency service and immediate availability cannot be guaranteed, and we may not always be able to attend right away.
If an urgent appointment is arranged, a full-service fee of £56 will apply to cover the additional support.
- 12. Terms of Service Agreement
By enrolling in the At Home Dental Care membership, you confirm that you have read, understood, and agree to the terms and conditions outlined. By signing into the membership, you consent to the membership and consent to these terms, and your first appointment will be scheduled accordingly.
